Diabetes affects more and more people each year, both nationally and right here in Western New York. Research has found that people with pre-diabetes – a condition in which blood sugar levels are high, but not yet high enough to meet the criteria for diabetes – can make lifestyle changes to reduce their odds of diabetes.

Our 12-month prevention program is designed for those diagnosed with pre-diabetes or who are at risk of developing type 2 diabetes. The program includes 16 weekly, one-hour sessions covering nutrition, exercise, and behavior modification. We then meet monthly for continued support and maintenance.

A referral from your physician is required. Click here to download the form. You do not need to be a member of the YMCA to participate. Fees are covered in full for Independent Health Medicare, Medicaid, and Commercial Health Plans. Fees are covered in full for Blue Cross Blue Shield Medicare and Medicaid Plans. To obtain a referral, please contact one of our branches.
